Athlete Participation Info – 2024-25

Dare County Schools has a new vendor handling athletic forms this year. Information on using this new straight-forward system can be found at the link below. Parents and their student-athletes can use Rank One to complete four online information forms and also view the status of the physical (which must be submitted to the school) that must be on file for each student to compete.

Find the info form here. It also includes a registration link.

In order to be eligible to participate in a Fall, Winter, or Spring sport at FFHS, you must meet the following criteria:

  • Have turned in a current physical form to the certified first responder or athletic director
  • Have completed our online Athletic Packet at Rank One (one time per school year)
  • Provide proof of insurance – either through personal/family plan or school-purchased plan
  • Have passed 5 out of 7 units for previous semester (year-long classes count as 1 unit; semester classes count as 2 units)

Athletic Physical Forms

NCHSAA Physical Forms are linked below. Download the forms your child needs at the following links and print them out to take to your physical appointment:

The North Carolina High School Athletic Association REQUIRES that student-athletes absent from athletic practice for five (5) or more days due to illness or injury shall receive a medical release by either a physician licensed to practice medicine or his/her designee (nurse practitioner, or physician’s assistant ) before readmittance to practice or contests.

As a reminder, any time your child is evaluated or seen by a physician for an illness or injury, a regular doctor's note is required to return them to athletics no matter the length of time missed.
